Whether you’re recovering from a heart attack, cardiac surgery, or managing chronic heart conditions, our outpatient services in Phase II and Phase III provide a safe, structured, and personalized path back to strength, confidence, and daily living.
Phase II – Supervised Recovery & Risk Reduction
Phase II is the early outpatient stage, typically starting shortly after hospital discharge. Under
close supervision by our experienced rehab team, we focus on:
● Monitored exercise sessions to safely rebuild cardiovascular endurance
● Heart rate, blood pressure, and ECG monitoring during sessions
● Education on heart-healthy habits, nutrition, medication adherence, and stress
management
● Risk factor modification to prevent future cardiac events
● Emotional support to manage anxiety, depression, or fear after a cardiac episode
Our goal during Phase II is to safely transition you from medical recovery to functional
independence.
Phase III – Maintenance & Long-Term Wellness
Phase III is a self-managed but professionally guided program for individuals who have
completed Phase II or are managing long-term cardiac conditions. It includes:
● Individually tailored fitness programs under professional guidance
● Progressive aerobic and resistance training to improve stamina and strength
● Lifestyle coaching to maintain heart health and reduce risk factors
